There are a total of 62 collectible locations in Outlast: 31 Documents & 31 Notes. Finding all collectibles will earn you the PULITZER gold trophy.
The Documents can be picked up like normal collectibles. The notes are obtained by filming certain events or objects with your camcorder. You have to collect everything in one playthrough to unlock the trophy! Your statistics will reset when you use chapter select or start a new game. Missing a collectible means you won’t get the trophy. As a precaution you should create new save games regularly. You can have up to 50 save games. That allows you to go back if you have indeed missed something.
You can see exactly which collectibles you have found by pressing the D-Pad left / D-Pad right buttons.
– Chapter 1: Administration Block –
Document #1 – The Whistleblower
Obtained automatically when arriving at the Asylum.
Note #1 – Mount Massive Asylum
After leaving the car, go through the gate and look at the huge Asylum in front of you.
Document #2 – Project Walrider Patient Status Report For William Hope
After climbing through a window you will be in a dark room. After the dark room you need to go through a corridor. Look in the second room on the left to find this document on a desk.
Note #2 – I’m Inside
After you go through the air vent you will drop down to the library. At the end of the library is a dying soldier on a stick. Look at him and the note will unlock.
Document #3 – Warrant for Seizure
After you are being attacked you will fall to the floor below and see a short cutscene. After you stand up there is a desk and some corpses in front of you. The document is on the desk.
Document #4 – Project Walrider Patient Status Report For Chris Walker
After picking up the previous document, go through the dark room to your right. There’s a well-lit chamber in the corner of the room. In there you will find the document.
Note #3 – Broken Men and Dead Television
Leave the well-lit chamber from the previous document and go down the hallway. At the end is a room where 3 men stare at a dead tv. Look at them with your camera.
Note #4 – The Witness
In the restroom next to the security room. Look at the corpse on the toilet.
Document #5 – Project Walrider Profitabilty Report
In the room at the end of the hallway, next to the security room. The document is on a desk in the corner of this room.
Note #5 – Big Fucking Guy
After accessing the security computer you are trapped inside the room and have to hide in a locker. Look at the big monster guy when he enters the room.
Document #6 – MKULTRA CIA Hypnotic Suicide
When you enter the downstairs area where you have to restore the power, you will have to squeeze through a gap (very beginning of downstairs area). Immediately look to your right and you’ll see this document on a desk.
– Chapter 2: Prison Block-
Note #6 – Father Martin’s Cell
After turning on the power and making use of the computer in the security room, you will see another short cutscene. This time you wake up in a padded room. Look at the blood on the right wall that says “Rest in Peace”.
Note #7 – Necromantic
Shortly after the last note you will have to squeeze through a gap in a wall and then climb up a ledge. There’s a guy in a straitjacket. Just keep standing there and look through your camera to unlock this note.
Document #7 – Father Martin, Finger Painter
When you have to follow the trail of blood, you will go through a disinfection chamber and then up some stairs. There’s a guy looking at a bloody wall. To the left side of him is the document (at the end of the corridor in the dark).
Document #8 – Purge Gate Maintenance Memo
You will come to a point where two naked men are standing behind a locked prison door. You will need to climb out the window to get around them. Once you reach the other side, jump through the broken window of the office room in front of you. There’s a document on the desk in this office.
Note #8 – Total Security
In order to open the door to the showers, you will have to retrieve a key. On your way to the key you will go down a very dark, long hallway and on the left you can see the big monster guy killing another inmate. Look at him with your camera to unlock the note.
Document #9 – Dr. Wernicke Death Certificate
After going through the Showers, immediately go into the room to your right. This room has two prison cells in it and the document is on a desk near the door.
Document #10 – Dr. Wernicke Obituary
After escaping the big monster guy, you will come to a large, well-lit room with many prison cells. Look inside the first prison cell to your right, after making the jump from the ledge (second floor).
Note #9 – Walrider
Once you’ve reached the top of the prison block you will have to drop down a hole to move on. You will walk straight into the showers — and there’s blood everywhere. Aim your camera at the bloody “Walrider” writing on the lockers.
– Chapter 3: Sewer –
Document #11 – The Gospel of Sand
Shortly after entering the sewers you will have to crawl through a pipe. Once you reach the other side, head left and you’ll find this document in a well-lit area.
Note #10 – The Doctor Is Dead
Shortly after draining the sewers and escaping the big monster guy, you will enter a room where a disfigured man greets you. Listen to what he has to say.
Document #12 – Still Bleeding
This one is a little tricky. You will come to a flooded, completely dark area. It’s literally pitch-black everywhere. Navigate through the flooded area until you see a dead man leaning over the handrail of a catwalk. Take a few more steps to drop down the catwalk. Now on your left you’ll see a well-lit area and in front of you is another broken catwalk. You need to climb up the broken catwalk in front of you and keep going until you find a door. Open it and you’ll see the document on the ground. This is just before you encounter the big monster guy in the flooded area.
– Chapter 4: Male Ward –
Note #11 – A Feast for Flies
Shortly after escaping the big monster guy, you will have to squeeze through a gap. On the other side is a sign that says “Male Ward” and at the end of the tunnel is a room filled with corpses. Look inside to unlock the note.
Note #12 – Organized Torture
After going up the stairs from where you acquired the last note, is a small container that you need to push aside to crawl through an air vent. The vent will lead you to a torture chamber. Exit the chamber and enter the very dark room on the left. Enable night vision and investigate the writing on the wall.
Document #13 – Project Walrider Cost Report
After pushing aside the next container, you will come to a hospital room. At the very end of this room, on the right side, is the document.
Note #13 – Dr. Rick Trager
After Trager tortured you and cut off your fingers, you will come to a room where another torture victim is lying in a bed. He will scream and Dr. Trager will kill him. Film how he is killed while hiding under a nearby bed and aim the camera directly at the torture victim.
Document #14 – Request for Reassignment
While you’re on your way to find the elevator key, you will have to crawl through a dark air vent. You enter the air vent after pushing aside the heavy container in the hallway. Once you drop out of the vent, go to the restroom in front of you and open the door on the right side to pick up this document.
Note #14 – Death of Trager
Immediately after Trager is dies in the elevator you need to use your camera and aim it at his head.
Document #15 – Persecutorial Delusions
After you meet the Priest, go into the dark hallway and make a right. You’ll come to a locker room. At the end of the room, in a dark corner, is this document.
Note #15 – Let it Burn
Film the man in the burning cafeteria. He will automatically start talking to you when you pass by. Congrats, you should now have unlocked the “EDUCATED” silver trophy!
Document #16 – Psychopathological Proximity Stress Disorder Pamphlet
After exiting the burning room, go left before squeezing through the gap. There in the corner of the dark room you will find this document.
Note #16 – Fingers
After activating the sprinklers you need to go back through the room that was previously in flames. Now go to the cafeteria and once you enter, a psychopath will attack. After he is gone, look for a side room in the cafeteria (left side of where you entered). In there is a bottle of fingers that you need to film.
– Chapter 5: Courtyard –
Document #17 – MKULTRA Program Excerpt
Before exiting the building (door with red Exit sign), go into the office on the right side. There’s a document on the desk.
Document #18 – Our Own Personal Calvary
Can be found outside, in the dark. After you go through the door with the red Exit sign, run in a perfectly straight line until you reach the end of the area. There are some stairs that lead up to the document. Behind it is some bloody writing on the wall that says “how alive are you”.
Note #17 – The Thing in the Dark
When you first go outside, your objective is to retrieve a key to open a locked door. After the door is open, keep walking down the dark hallway and a ghost will come to scare you. Keep the camcorder in nightvision mode ready and film the ghost.
Note #18 – Don’t Drink the Water
After escaping the big monster guy, you will have to go through a short tunnel. Now turn right and look for a fountain full of blood and corpses. Film the bloody fountain to unlock this note.
– Chapter 6: Female Ward –
Note #19 – The Sound in the Machine
Very shortly after entering the female ward you will have to squeeze through a gap and climb through a window. After climbing through the window, a spinning machine will be in front of you (it makes some metallic noise). Get closer to it to unlock the note.
Document #19 – Walrider and Nightmares
After finding the last note you will go through a well-lit room and reach a long, dark corridor. As soon as you enter this long corridor, turn right and go all the way to the end until you find this document under the stairwell.
Note #20 – No Safe Place
After leaving the dark hallway from document #19, you will come to a very well-lit room with a broken wheelchair in the middle. There’s an elevator in the same room. Inside the elevator is a corpse which you need to film.
Document #20 – Project Paperclip Excerpt
After making it through the long, dark corridor from the last document, you will reach a well-lit room with a broken wheelchair in the middle. There are two stairs: one going up and one going down. For this document you need to go down and look in a corner of the basement.
Document #21 – You Promised Me a Rose Garden
After retrieving the key and jumping up the broken stairs, you will come to a point where the floor breaks in. The Walrider Ghost will then appear. Follow it and enter the next room. In the corner of this room, on a bed, is this document.
Note #21 – Lies
From where you go the last document, turn around and leave the room. You’ll see the Walrider Ghost again. Follow it and you’ll see a well-lit room on the right side with bloody writing on the wall. Film the bloody writing.
– Chapter 7: Return to Administration Block –
Document #22 – Billy’s Dreams
As soon as you enter the “Return to Administration Block” area, the big monster guy will show up in a long corridor in front of you. You need to enter a door on the left and climb up to an air vent. While in the air vent, make a right and explore the hidden side room to discover this document about Billy’s Dreams.
Document #23 – The Spirit Breach
After obtaining the previous document, go back into the air vent and push aside the corpse that is blocking your path. Drop down and you’ll find yourself in front of the library. This document is in the library, on a computer desk.
Note #22 – The Wernicke Exit Interview
When you enter the theater, a film will start to play. Capture the film with your camera. It takes a minute until the note is unlocked, stay in this area and look at the screen the entire time.
Document #24 – Necrotizing Fasciities
After leaving the theater you will have to go through a corridor. Go to the very end of the corridor into a well-lit room to find this document on some boxes.
Note #23 – Static Prayer
After using the key to unlock the closed door, go up the stairs, through the kitchen and you will reach a large dark room with a patient looking out the room. After making your way through this room, you are in a hallway. Enter the first room on the right side to find a praying patient in front of a TV. Film him.
Document #25 – The Gospel of Judas
In room B329 of the corridor where all the praying patients can be found. It’s shortly after squeezing through a gap in the wall, but before climbing out the window in room B326.
Note #24 – The Passion of Father Martin
Film Father Martin’s Death in the church.
– Chapter 8: Underground Lab –
Note #25 – Under the Mountain
Film the first room of the underground base, after the elevator ride to get this note.
Document #26 – The Pride of Wisdom
Same room as Note #24, behind the front desk.
Document #27 – Variant Postmortem
After you have obtained document #26, leave the room and go through the tunnel. You’ll now enter a hallway with two labs on the left side. The second lab contains the document.
Note #26 – Morphogenic Formula
After leaving the hallway from document #26, you will come to a very long tunnel. Enter the first door on the right side to find a secret lab. Film the Morphogenic Formula written on the board.
Note #27 – Death of the Soldier
Unlocked automatically.
Document #28 – Gods and Monsters
After your talk with the mad scientist, you will be chased by a ghost. Escape and go through the decontamination chamber upstairs. The document is in the next room you enter, on the front desk.
Note #28 – Wernicke’s Machine
After the room where you picked up document #28, you will come to Wernicke’s Machine. It looks like a gigantic orb. Film it to unlock the note.
Document #29 – Morphogenic Engine Chamber Precautions
Same room as note #27. The document is on the servers in front of the big orb machine.
Note #29 – Billy Hope
This is just two meters to the left of document #29. Film the guy in the glas orb.
Note #30 – Life Support
After leaving the room where you found note #28, you will go through a tunnel and enter a room with some blue tanks in the middle. Film the tanks for this note.
Document #30 – The Modern Prometheus
Same room as note #29. Go up the stairs and look inside the small room, on the desk.
Document #31 – Permission to Proceed
After going through the decontamination chamber and escaping the Walrider ghost, go up the stairs and you will see the final Outlast Document on the desk in front of you.
Note #31 – Death of Hope
After the cutscene where the Walrider Ghosts attacks you, film the bloody life pod where Billy was previously being held to get the final Outlast Note.
