Starfield contains 111 Star Systems. Visiting all Star Systems unlocks The Stars My Destination Achievement. All Star Systems can be viewed by fully zooming out on the Starmap. They are all available from the start.
They can all still be visited until reaching the game’s Ending (before finishing Main Mission: One Giant Leap). After the ending it automatically starts New Game+ and resets all your exploration progress. Any discovered Star Systems will be reset so be sure you keep a Manual Save before the ending and collect them all before entering New Game+.
The furthest distance between two stars is 28 Lightyears (LY). You will need a spaceship that can jump at least 28 LY distance to visit all Stars. The Jump Distance of your ship can be seen in the Ship Menu. Press / Tab > in bottom left click the ship > on left side it will show “Jump LY” (LY stands for Lightyears distance).
You can increase the Jump Distance by buying a new ship, installing a stronger generator, or buying the “Astrodynamics” skills from the science skill tree. Your starting ship won’t be good enough to reach everywhere. Most ships that have a base stat of 25 Lightyears can be upgraded to 28 Lightyears by installing a stronger Gravdrive and Reactor. Or simply buy a ship that starts with 30 Lightyears and you’ll be good to go. Alternatively, you can steal a Spaceships to find one with enough jump distance.
Picking a ship with a lot of fuel storage will allow you to jump more stars in one go without stopping. Fuel automatically recharges instantly after each Grav Jump, it’s for free and doesn’t cost anything and doesn’t require any resources. Keep in mind some Star Systems will have immediate enemy encounters so it’s also important to have good weapons and shields. Or you can try to quickly jump to the next Star to evade the enemies. Playing on the lowest difficulty and buying the Tech skills for better Shields/Damage will help if you get into spacefights with high-level enemies.
If you can’t travel to a star it’s because you haven’t discovered the “route” to it yet. You must have visited all star systems in the chain leading to the destination star. Example: you want to jump somewhere 3 stars away but have never visited the 1st and 2nd star system sitting in between. In this case you would need to manually jump to the 1st star, then the 2nd star, then the 3rd star. Then they have all been discovered and you can jump across all 3 in one go next time.
You can tell which Star Systems you’ve already visited if they show a “Survey %” in the top left corner when you scroll over them on the Starmap:
If they don’t show the Survey % it means you’ve not visited the Star System before and still need it for the achievement. Simply scroll over each dot on the Starmap to see this.

I got this achievement when the counter in Status/Exploration hit 120. So I believe there should be 120 star systems in total. Also, already-visited star systems are shining in the Starmap so unvisited ones should be obvious.

Just wanted to jump on this comment and confirm there are in fact 120 solar systems that must be visited. When you scroll over the systems you’ll notice some dots have more than one name on it.

Where can I buy a grav drive for 28LY? can’t seem to find one.

You need to buy a new ship from Service Technicians, found at every landing pad of populated cities. Pick one with 30LY drive installed.
Can also upgrade some ships from 25LY to 28LY by changing out some parts in ship builder. Or just steal a ship and hope to get lucky that one has a good enough drive (see How to Steal Ships).
