Brookhaven Hospital (Otherworld) is the 6th area in Silent Hill 2 Remake. This 100% walkthrough will guide you to all collectibles in chronological order. Everything that’s needed for trophies, achievements and 100% completion is included: Memos, Strange Photos, Glimpses from the Past, Key Items, Puzzle Solutions, Trophies & Achievements.
All Collectibles must be found in a single playthrough. Everything is missable! After leaving an area you can’t go back. There’s no Chapter Select and no Free-Roam after the story. Collectibles don’t carry over into New Game Plus either. The game has 10 manual save slots, make use of this by saving in different slots regularly. If you missed anything you can reload a previous save to replay from there.
The Memos and Strange Photos you found can be viewed in the Inventory Menu by pressing the Touchpad. Glimpses from the Past can’t be tracked anywhere. This guide is intended for the “Light” or “Standard” Puzzle Difficulty, the solutions for both difficulties will be mentioned. Combat difficulty does not matter, all Trophies can be done on “Light” difficulty.

Memo #38 (Brookhaven Hospital): Evaluation Card, Patient #3141
On a wheelchair immediately in front of you after going through the first door, in the Dayroom.
Memo #39 (Brookhaven Hospital): The Procedure
When you reach the room with the Chained Box, take the western path through The Nurses’ Station. Continue until you go up some stairs to the third floor. This note will be on a table in the X-Ray Room.
Key Item: Instrument of Force
In the next room, on a medical table next to a mannequin in a chair.
Key Item: Key of Bliss
After going down a ladder to the Kitchen, go through Storage/Medical Records to find this in a hole in a wall.
Key Item: Lapis Eye Key
Return to the mannequin and use both items on it to get the key.
Warning: Point of No Return
Using the Lapis Eye Key on the Chained Box will lock you out from the western path of the Hospital (Otherworld). Make sure you have everything there before you use it.
Memo #40 (Brookhaven Hospital): Calendar Page
Return to the Chained Box and take the southern path. Enter Room M3 and smash the wall to find this memo on the floor. This memo contains the hints for the first code for the Chained Box.
Glimpse of the Past #16
On the far east wall of Room M5, there are handprints you can examine.

The first digit is the number of years in the first line of the memo, the second digit is the number of pills in the sink, the third digit is the time on the clock in Room M1 when you adjust it, the fourth digit is the number of flashes of the light when looking through the hole: 1622 (Light + Standard Puzzle).

Using the 1622 code on the Chained Box will lock you out from the southern path of the Hospital (Otherworld). Make sure you have everything there before you use it.
Memo #41 (Brookhaven Hospital): Patient’s Note
Return to the Chained Box and take the eastern path. Break the wall into the Laundry Room and move the trolley so you can climb into Room L3. The note can be found on a bed in the Observation Room through the next door.

On the floor in the same room, in front of the static television.
Strange Photo #14: “No one knows”
In the bedside table of Room C5, through the hole directly in front of you.
Memo #42 (Brookhaven Hospital): Manic Scribble
On the bench inside the Director’s Office.
Memo #43 (Brookhaven Hospital): Nurse’s Note
Go up the ladder in the office to Room I7, where you will find the note on a small table in the corner.
Memo #44 (Brookhaven Hospital): Director’s Note
Follow the red cable to the generator to power it on, then crawl through the hole to find this memo on the floor in the locked restroom.
Code for Chained Box
Return to Room I7 and interact with the machine to reveal the code: 9659 (Light + Standard Puzzle).
Warning: Point of No Return
Using the 9659 code on the Chained Box will lock you out from the eastern path of the Hospital (Otherworld). Make sure you have everything there before you use it.
Key Item: -1 Elevator Button
Return to the Chained Box and open all three locks: Lapis Eye Key, 1622 and 9659 (Light + Standard Puzzle).
Key Item: Copper Ring
Make your way through the basement until you reach the Pump Room, where you will find the ring on a crate.
Strange Photo #15: “They mustn’t know”
Continue upstairs to the first floor and you’ll go through the Pool area. On a bench at the back is this photo.
Key Item: Lead Ring
Interact with the freezer in the Doctors’ Lounge to find the ring, accessed through Shower Room.
Memo #45 (Brookhaven Hospital): Dissociated Notice
On a bench in the Locker Room, which is the next room.
Info: There is a jeweled box in the Pharmacy you can open with the questions from the Elevator Quiz, it contains health and ammo. The answers are Q1 = 2, Q2 = 3, Q3 = 1 (Light + Standard Puzzle), if you get any wrong you cannot retry it and will take damage.

Using the rings on the door will lock you out of the rest of the Hospital (Otherworld). Make sure you have everything before you open it.
Memo #46 (Brookhaven Hospital): Ominous Note
After going through the door with the two rings, you’ll be on some stairs. Head down one landing to spot a note on the floor. Highly Missable! Make sure you grab it before going all the way down the stairs.
