Mortal Shell has 19 Inscriptions and 17 Etchings (36 Collectibles total) needed for the Seeker of False Truths trophy/achievement. This guide shows all their locations. After beating the story you can choose to stay in your current playthrough to keep exploring and pick up your missing items and collectibles (except for 3 missables, see warning below). Lore can also still be collected in NG+ and progress on viewed lore is kept, the trophy/achievement is awarded when all entries are filled in the Compendium.
Missable Collectibles: The three etchings located in the Seat of Infinity region cannot be picked up if you have defeated the boss in that area during the current playthrough. If you have missed them you will need to obtain them in New Game+. Everything else can still be found after the story in free-roam by staying in your current playthrough.
Inscriptions
Inscriptions are lore pieces that can be found around the game environment on objects. These give game background and info on each area.

In the Fallgrim area at the start of the game, go left in the open forest area. In between a split in the path, you can find a white stone statue with this inscription.

In the Fallgrim area at the start of the game, go towards the right into the area with the stone pillars. On the left, just up the path, you can find this inscription.

From the location of Inscription #1, turn left and go into the graveyard area. To the left of the large tree, you can find this inscription.

In Fallgrim Tower, facing Sester Genessa, go out the door to the right, then turn left and go straight forward to find this inscription.

In Crypt of Martyrs, in the Abandonded Chamber area, follow the path forward to the room with the passages leading out of it, then crawl through the hole on the right side. This inscription is to the immediate left.
- Inscription – The first vision came upon me like a waking dream…
In Crypt of Martyrs, in the Enshrined Sanctum area, from the start where Sester Genessa is, go forward then take the staircase going down to the left to find a dead end with a chest and this inscription. Note the inscriptions in this area are these headless bodies holding slabs.
- Inscription – Kneel before the Immaculate…
In Crypt of the Martyrs, in the Martyr’s Altar area where you would drink the nektar to go to the boss room, go all the way to the back left of the room to find a crawl hole, and on the other side a room with a chest and this inscription.

In Seat of Infinity, in the Eternal Narthyx are, this inscription is on a platform above the right side switch you activate to progress. To get there, go to where the third switch is, then go all the way to the back wall near where the large trees are. Here there is a ramp you can go up to the long platform suspended between the pillars. At the end, you can find this inscription on the ground near a bunch of slabs.

In Seat of Infinity, in the Dim Gate area, from the Sester Gennesa point go all the way left at the crossroads to the dead end. On the right side in one of the alcoves you can find this inscription in front of a large statue.

In Seat of Infinity, just before the Shifting Archives area, you can find this inscription directly in the middle of the path before entering the structure with the stacks of slabs.

In Seat of Infinity, in the Shifting Archives area, from the Sester Genessa point follow the wall left to the final section of stacks before going back into the section with lets of stacks. You can find this on the ground left of a couple of tall stacks.

In Seat of Infinity, in the Shifting Archives area, from the Sester Genessa point turn around, then go one section to the right then to the opposite side. This inscription is on a slab hiding behind some shorter stacks of slabs.

In Seat of Infinity, in the Shifting Archives area, from the Sester Genessa point go to the right towards the large face. This inscription will be the final slab on the ground on the left side.

In Shrine of Ash, in the Sanctum of Ash area, go down the stairs into the lower area, and you can find this inscription on the right.
Etchings
Etchings are secret lore piece found in the game, that are revealed by attacking walls with your weapon to reveal them. When you find one you also obtain a Glimpse of Wisdom.
Note: The 3 etchings in Seat of Infinity cannot be obtained after you defeat the boss in that area. You need to collect them before beating the boss, or find them in NG+.

In Seat of Infinity, in the Dim Gate area, go all the way up the tower to the top where you can find the two teleporters, one of which leads to a shortcut elevator, and the other leads to platforms you can drop down for a Quenching Acid. Go through the one leading to the platforms you can drop down, and this etching will be on the wall on the left just before the hole. This must be picked up before you defeat the boss in this area, or you need to pick it up in NG+.
