In Main Quest 6: For Whom the Bells Tolls of Kingdom Come Deliverance 2, you have to find out what medicine to give to Captain Thomas. Curing Thomas is mandatory for completing the quest’s overarching objective to save Sir Hans from execution.
Step 1: Reach Captain Thomas
He’s upstairs in the east-wing of Trosky castle. You can sneak up there. There’s only one guard at the stairs, can throw a stone to distract him. To gain access to the east-wing you must first help the blacksmith to make horeshoes.
Step 2: Make Fever Tonic to cure Captain Thomas
Head to the top of the west-wing of Trosky Castle. There will be guard in the kitchen but you can give him food to make him go away. Food is found directly under the kitchen in the bathhouse. Near the top (two floors below the top) you find an alchemy bench. In the room with the alchemy bench collect the Physician’s Journal from the left shelf. It reveals that you must make Fever Tonicum.
Then lockpick the easy difficulty chest in the room that contains the ingredients for the potion. Lockpicks are obtained by helping the Stablehand Hired Hand Kabat to get back his rosary (can pickpocket it from Father Nicodemus).
Then brew the Fever Tonic at the alchemy bench:
- Pour wine into cauldron (top left shelf, red vase)
- Take 3 handfuls of feverfew and put them into cauldron
- Move down Cauldron (

)
- Boil it for 2 turns (turn sandglass,
/
to use bellows to heat up the flame, turn sandglass again and let it run out)
- Take 1 handful of elderberry leaves, put in mortar and grind with pestle. Dump in cauldron when done.
- Add 2 handfuls of ginger into cauldron.
- Take phial (bottom left side of alchemy bench)
- Use the Still (bottom left side of alchemy bench) to distill the potion.
Afterward return to Captain Thomas to give him the tonic. At the same time you’ll get a cutscene where you save Sir Hans, which ends the quest.
