Starting Location: Bloodletting
This quest is part of Sugi’s questline. You must have finished Side Tale “Troubled Waters” for this quest to become available. It will then be marked on your map automatically.
Meet Sugi by the old fishing village
When you arrive at the quest marker, you’re supposed to meet Sugi there. But instead, all you find is a group of enemies. Defeat them.
Free the captive
After killing the enemies you need to free the hostage on the boat dock.
Speak with Sugi
Then Sugi will appear, talk to her on the boat dock.
Defeat Yamaneko’s lookouts
Across the river, just 50 meters away, are 3 archers on a hill. Climb up there and defeat them.
Sit and Wait
In the same spot where the archers were standing is a campfire. You need to sit down there to skip ahead to nighttime.
Survey the camp
After waking up, you’ll automatically enter “Survey Mode” and must look at 3 objects. White rings on the screen will show you what direction to look in.
#1 – Left side, Boat at the beach
#2 – Center, Campfire behind an enemy
#3 – Right, Campfire on some rocks in the distance
Signal Sugi’s Hwacha
Now you must climb down to the beach and light the campfires by pressing at them. You can sneak there and ignore the enemies or stealth kill them. Below are the locations of all 3.
#1 – At the beach, behind an enemy.
#2 – On rock in the distance, climb up and defeat the enemy that’s also there.
#3 – Follow the cliffside next to the 2nd campfire and then jump across to a grappling point and press to hang on to it and pull yourself up. There are a bunch of enemies here, if you get spotted you can take them down quickly with your bow.
Defeat the Mongols and Yamaneko’s Crew
After lighting the third campfire, both enemy factions will start fighting each other at the beach. Climb down and defeat them all.
Regroup with Sugi
After the battle, follow the shoreline north. There you’ll see an interaction point and the burning ship in front of you.
Examine the Debris
This will skip some time and you’ll see Sugi’s head being washed ashore. It seems as if she died in the burning boat and the quest officially ends here. But there is more to the story and another secret step you can do to get Sugi’s hat!
How to Get Sugi’s Kasa Hat
To unlock Sugi’s Red Hat you must return to the village where you first met Sugi. It’s the village called Thieves Rest in the south-east edge of the map (should have a fast travel point unlocked there). In the village you can find people gathered around a corpse pinned to a tree. The corpse has a note stuck to its chest that you must read. It reveals that Sugi “faked” her death and went away as she thinks it’s safer for everyone without her being there. This unlocks Sugi’s Hat.
Below are images of the exact map location where that corpse is found and the item unlock screen.
This finishes the Bloodletting quest in Ghost of Tsushima Iki Island.

Daniel says
I got the hat by just completing the mission … didn’t have to go back for the village
Rdks says
..the chest that you examine just before you meet Sugi is locked – is there ever access to UNlocking it?
PowerPyx says
I never found a way to unlock it, pretty sure it’s supposed to stay locked and this is normal.
