Ghost of Tsushima has 40 Record Locations. Finding 20 Records unlocks the Avid Reader trophy. This guide shows all their locations.
None of the Record Collectibles are missable! It doesn’t matter what dialogue choices or what ending you pick during the story. You can still get all of them after the story in free-roam. You can reveal them on the map by wearing the Traveler’s Attire (given to you for free by talking to merchants). After finding your first Record, press on the world map and select the wind for Undiscovered Records. The wind will then lead you to the remaining Records and the Traveler’s Attire will make the controller vibrate when you get close to the Record (upgrade this armor to increase the effect and enable controller vibration).
Most of these Collectibles are found at settlements that you liberate during main- and side tales. If you’ve already burned down all enemy strongholds you can still find the Records there later, they’re not missable.
You can keep track of your progress in the pause menu under “Collections” > “Records”.

It would really help if the records have titles/names.

I’m pretty sure that specific records aren’t bound to specific locations, so you won’t always find the same record in the same place

Conversations with the Khan 11 doesn’t show up until after you do Story Quest in Izumi Village, it’s the last Record i need but Guiding Wind goes nowhere but I’ve been using a Interactive Map. Also Records are not in random Locations. Use Guiding Wind once you unlocked all Locations.
