There are 5 Mako Frequency Locations in Final Fantasy VII Rebirth. Finding all Mako Frequencies is needed for side quest From Whence Life Flows, to complete Objective “Measure the Mako Frequency”. They aren’t marked on the map which makes them tricky to locate. They are all found in Cosmo Canyon region.
You are given a map/image by the quest giver that can be viewed with 
and holding 
to zoom in. This will show where on the world map the location is. It’s highly recommended to fully unfog Cosmo Canyon region first. You will also want to unlock the Chocobo in this region first by completing Side Quest: Bonds of Trust (the chocobo is needed to fully explore the map). If you first finish the Cosmo Canyon World Intel you will have the full map unfogged and can fast travel everywhere.
Mako Frequency 1 Location
The first Mako Frequency is directly east of Activation Intel 3. Open the gate for a cutscene.
Mako Frequency 2 Location
The second Mako Frequency is found north-west of Divine Intel 1.
Make Frequency 3 Location
The third Mako Frequency is slightly west of Phenomenon Intel 1.
Make Frequency 4 Location
The map for the 4th Mako Frequency is confusing at first, it’s rotated by 90°. The compass in the top left of the map shows where north is.
The 4th location is slightly north-east of Activation Intel 4.
Make Frequency 5 Location
The final map is upside-down.
The 5th location is on the mountain where Expedition Intel 4 & Moogle Intel are. You can get up there by going south of Activation Intel 4 until you reach the southern edge of the map, then go up the mountain path from the south-west edge of the mountain. Jump from one ramp to the next using the chocobo until you reach the very top of the mountain.
Head through the final gate, then defeat the enemies and watch the final cutscene to finish Side Quest: From Whence Life Flows. For more guides check out Final Fantasy VII Rebirth Wiki.
