In Elden Ring there’s a secret region in the north of the Mountaintops called Elphael, Brace of the Haligtree and another one called Miquella’s Haligtree. It’s a secret high-level endgame area containing some of the toughest bosses in the game, as well as some legendary gear needed for several trophies and achievements.
This is the area of Miquella’s Haligtree and Elphael:

Step 1: Get Haligtree Secret Medallion (Right)
Go to Lirunia of the Lakes: Village of the Albinauries. It’s in the south-west corner of Liurnia region. Past the site of grace is a fire-throwing enemy, go straight after him to find a barrel. Touch the barrel and it will turn into an NPC, talk to him to get Haligtree Secret Medallion (Right).
Step 2: Get Haligtree Secret Medallion (Left)
This requires you to first reach the central part of Mountaintops of the Giants. To get there, you must defeat the main boss Morgott, the Omen. He is one of the shardbearers and is mandatory to advance the story. You find him in Leyndell, the Royal Capital, in the upstairs section in the south of the city. See the boss video below if you haven’t defeated him yet:
He grants you a medallion piece. Head to Avenue Balcony site of grace in Royal Capital, and from there go up the stairs and follow the path to the east (would be to your left) until the end. There interact with the big gate, leading you out of the city and to a big wooden elevator. Use that elevator and then go across the city walls until you find another big elevator that takes you to the Forbidden Lands. From there follow the linear path until you get to Grand Lift of Rold and use the lift (requires Medallion dropped by Morgott). Remember this lift, we must go back there later and finding the last Medallion Part. The lift will take you to the central Mountaintops. Head north and drop down the cliffs to reach Castle Sol. Defeat Commander Niall at the end of Castle Sol, then follow the path behind him to find the Haligtree Secret Medallion (Left).
Step 3: Use the two Medallion Parts at Grand Lift of Rold
Now backtrack to the Grand Lift of Rold. Press 
/ 
to switch to the Haligtree Secret Medallion and show it at the lift. This will bring you to Mountaintops West.
Step 4: Ordina, Liturgical Town
Head north to Ordina, Liturgical Town.
In the back of the town you find an Evergaol with a statue next to it. Interact with the statue, it tells you to light some flames. Then step into the Evergaol to see a different version of the town. There are now 4 statues around the town that you must light. One is around the center of the town on the street level. The other 3 are all on rooftops on towers that you must climb via ladders. Be careful with the archers on the rooftops, they hit very hard. After lighting all 4 statues you will automatically leave the Evergaol – if you die you can retry by stepping into the Evergaol again. Step 5: Step through Portal in Ordina
Now that all statues are lit, the barrier up the stairs next to the Evergaol in Ordina will be removed. Head upstairs to find a teleporter, step through it. This teleports you to Miquella’s Haligtree.
Step 6: Go through Miquella’s Haligtree
You will arrive at Miquella’s Haligtree and must follow the roots. At one point an enemy will shoot glowing ball projectiles at you, which you must dodge through, then head down a ladder on the left. This leads to some buildings, follow the path until you reach the boss Loretta, Knight of the Haligtree. Defeat her and use the next elevator to enter the hidden city of Elphael!
That’s all there is to get to Miquella’s Haligtree and Elphael.
