- Bring Elena the medical ingredient
- Follow Lady Elena
- Investigate the Gracious Hand
- Report back to Lubomir
- Gather evidence and apprehend the abbess
Starting Location: Saint of the Slums
To start this quest, you first need to witness a short scene occur in the Slums in front of the Gracious Hand building. Two people will be arguing with one of the sisters, after which they leave. You can then speak to the woman and agree to help to start this quest.
Bring Elena three miasmite, as she requested
Elena needs three Miasmite to make some medicine. If you’ve not done any exploring at night yet, Miasmite is a unique material that is dropped by Phantom enemies that only appear at night. Head out of Vernworth and you should eventually see greenish clouds floating around, which are the Phantoms. They are hard to hit as melee classes but are rather weak, so having at least one Pawn who is either magic or ranged should make taking them out easy enough.
Bring Elena the medical ingredient
Once you have obtained three Miasmite, head back to Elena in the Slums and turn them in.
Follow Lady Elena
Elena asks if she can show you around as a reward. Agree and then follow her into the Gracious Hand building.
Investigate the Gracious Hand
The sick man inside asks you to investigate the Gracious Hand. Go outside and you should be immediately stopped by a woman named Lottie, who tells you about the basement.
Report back to Lubomir
After speaking with Lottie, go back inside and speak with Lubomir again. Agree to investigate further.
Gather evidence and apprehend the abbess
You now need to find evidence of any wrongdoing. You can enter the basement using the stairs to the left when you enter the building. Follow the path through the second door into the room with all the sick people on beds, then through the curtain into the small room with a bunch of items. The item you need to pick up here is the Records of Treatment text on the bookshelf left when you enter, along with the Unlabeled Medicine item above the table. You can go show both of these items to Lubomir if you wish for some additional information.
With the Records of Treatment info you get a new objective to find two prior patients, who can be found at Walter’s Tavern further east in the slums.
They’ll likely be sitting near the fire in front of the tavern. Speak with them both.
Bruno tells you some more interesting information about Elena, and you receive another objective in the square in the Common Quarter area of Vernworth. Wait until night time before heading there, where there will be an automatic scene of her interacting with someone suspicious.
If you haven’t already encountered him, showing Lubomir the medicine will prompt him to tell you of a doctor named Radcliff who is in Checkpoint Rest Town. You can reach this quickly by taking the Oxcart on the west side of Vernworth.
After giving him the medicine he needs to time check it, so you need to wait for a day to pass, either by doing other things or just waiting on a bench or resting. Once he’s done return to him for info on the medicine. When you go back to Vernworth Lubomir will have been moved to the basement. Elena is waiting out front of the door to the sick room now, and you need to grab her with to turn her in, which completes the quest.
This finishes the Saint of the Slums side quest in Dragon’s Dogma 2.

Manny says
After the completion of the quest the new abbess Lottie will express concerns as to how to care for all the patients but will do her best, all patients will die in 3 days, visit the doctor in checkpoint rest town and he will request to be paid before going in and this will save Lubomir and other patients from death

Why the hell do you get no prompt for finding a way to save them?! Lubomir wouldn’t say anything after the quest complete and so I thought there was nothing more to do. You’re awesome for leaving this tip.
