Diablo 4 has 38 Side Quests in the region of Fractured Peaks. Each Side Quest will grant 30 Renown for the region, as well as various rewards. Some quests will require you use a gesture. Most quests are available from simply speaking to a person marked by a blue ! on the map. Some will require previous quests to be completed before they become available, and some will start upon picking up a particular quest item that can drop in certain areas. 3 Side Quests were added after release.

Desolate Highlands
1 – Menestad Coffers
Inside Kyovashad, available when first arriving. Head to the marked area in Sarkova Pass and search for the Monk.
2 – Raising Spirits
Inside Kyovashad, available when first arriving. Go to the soldiers and use the Cheer gesture.
3 – The Sealed Door
Inside Kyovashad, available after progressing Act I. Head to the marked area in Frigid Expanse and kill the Death Priest.
4 – The Woodsman of Nevesk
Inside Nevesk, available after first visiting Kyovashad. Head to Boulder Ridge, search the marked area for the axe and kill The Woodsman.
5 – Malady of the Soul
Inside Kyovashad, available after progressing Act I. Enter the cellar and kill Xul’Goth.
6 – Depths of Despair
Inside Kyovashad, available after completing Malady of the Soul and leaving the city for a bit. Search the marked area for Vasek and enter the Kyovashad Sewers, then head to the end and kill Demotath.
7 – Faith in Blood
Inside Kyovashad, available after completing Depths of Despair and leaving the city for a bit. Head to Margrave to collect the chalice and fill it by killing enemies in the marked area. Return to Margrave and then the cellar in Kyovashad to kill Demotath again.
8 – Sight to Madness
Inside Kyovashad, available after progressing Act I. Conquer Nostrava Stronghold, then enter Cultist Refuge dungeon and kill Theya.
9 – Blood of the Mother
Inside Kyovashad, available after completing the campaign. Head to Nevesk and speak to Voszalko, then search for clues. This quest was added after release.
10 – Shroud of the Father
Unlocked automatically after completing Blood of the Mother. Head to Mercy’s Reach dungeon and search it for The Child’s Caul in the last area, then return to Voszalko in Kyovashad. This quest was added after release.
11 – Severing the Bond
Unlocked automatically after completing Shroud of the Father. Enter Elysiuma Archives and interact with the chalice. This quest was added after release.
Dobrev Taiga
12 – A Cold Faith
In the Southeast Foothills, available when first arriving. Follow the quest marker to Margrave and speak to the man.
13 – The Cleansing Flame
Inside Margrave, available when first arriving. Head to The Cleansing Flame at the marked area and activate it, then kill all the monsters.
14 – Bound by Blood
Inside Margrave, available when first arriving. Follow the quest markers into Cutthroat’s Escape and kill Outlaw Sharpshooter at the end.
15 – Legacies of Light’s Watch
Inside Margrave, available when first arriving. Enter Light’s Watch dungeon and acquire the three journals.
16 – Better Days
Pick up Faded Old Painting, which can drop from searchable corpses east of Margrave. Bring it to Zalan Coste. May require completing Legacies of Light’s Watch.
17 – Unwritten End
Pick up Merchant’s Ledger, which can drop from Bandit enemies east of Margrave. Bring it to Zalan Coste. May require completing Legacies of Light’s Watch.
Gale Valley
18 – Ravenous Dead
Inside Yelesna, available when first arriving. Kill Ghouls north of Yelesna and collect 15 Ghoul Hearts.
19 – Traveler’s Prayer
Inside Yelesna, available when first arriving. Go to the shrine and use the Thanks gesture.
20 – Unyielding Flesh
Inside Yelesna, available when first arriving. Go to Gnarled Timbers and approach the chained person, then follow the woman and kill Yulia.
21 – Icebound Geode
Pick up Icebound Geode, which can drop from Ore Veins in Gale Valley after completing the Campaign. Bring it to Kratia.
22 – Righteous Idol
Inside Yelesna, available after completing Better Days and Unwritten End for Zalan Coste, and completing the Campaign. Purchase the Carved Wooden Knight from the merchant and take it to Zalan.
Sarkova Pass
23 – Gold Well Spent
Inside Menestad, available when first arriving. Head to the quest marker and kill Luskas.
24 – Chain of Possession
Inside Menestad, available after completing The Woodsman of Nevek. Enter the cellar and kill Magdalena.
25 – Legacy Unmade
Automatically started after finishing Chain of Possession. Head to the quest marker in Crags of Ill Wind and kill The Woodsman again.
26 – Forgive Me
Inside Kor Dragan Barracks dungeon, pick up Bloodied Note that drops from the elite enemy Julek. Give the note to Roza in Menestad.
The Pallid Glade
27 – Hungering Bone Cache
Pick up Hungering Bone Cache that drops from searchable corpses in Frostbite Trails. Head to the marked area and kill werewolves to harvest death anima.
28 – Frostbloom
Pick up Frostbloom, which can be found randomly when harvesting herbs anywhere in The Pallid Glade. Give it to Veroka in Kyovashad.
Frigid Expanse
29 – Cries of Innocence
In Olyam Tundra, available when first arriving. Enter Black Asylum dungeon and kill Corik Trost.
30 – The Dread Martyr
In Olyam Tundra, available when first arriving. Enter Hallowed Ossuary dungeon and kill Lord Avitus.
31 – Secret of the Spring
In Kylsik Plateau, available when first arriving. Head to the quest marker at Trough of Orobas and use the Wait gesture (will need to customise and assign it).
32 – Exorcist’s Cache
Pick up Sealed Exorcist’s Cache that drops from Undead enemies within The Deep White. Kill undead in the marked area to collect spirit anima.
33 – Gory Display
Pick up Warrior’s Chain that drops from a Khazra ambush in the east of Olyam Tundra, outside the entrance to Malnok. Take it to Bear Tribe Refuge.
Seat of the Heavens
34 – Shattered Tribute
Inside Bear Tribe Refuge, available when first arriving. Enter Hoarfrost Demise dungeon and kill khazra for 25 Ice Clan Bones.
35 – Hammer of the Champion
Inside Bear Tribe Refuge, available when first arriving. Conquer Malnok Stronghold, then enter Rimescar Cavern dungeon and find the Grizzly’s Maul on an impaled corpse.
36 – The Beast’s Challenge
Inside Bear Tribe Refuge, available when first arriving. Head to the marked area and kill Kauller the Collector, then return.
37 – Call of the Ancients
Inside Bear Tribe Refuge, available after completing the above 3 Side Quests. Enter the Hallowed Glacier dungeon and kill Cyhrach the First Born at the end.
Kor Valar
38 – Blood Sermon
Inside Kor Valar after completing Pilgrimage in Act I. Enter Sanguine Chapel dungeon and kill the Blood Bishop at the end.

I assume it can drop from enemies after doing the previous coste missions, I’m not sure though as I haven’t done it yet.

This is annoying, I am not sure which one I missed, all town quests seems to be completed. I missed 1 but I am not sure which one.

Chances are if you’re missing one and it’s not available in any of the towns/start locations, then it’s probably one of the items that drops from enemies/searchables, like the caches or frostbloom.

FYI If someone is struggling with the Icebound Geode quest. It wasn’t the case in the beta, but on the release, it’s mandatory that you FIRST end the story campaign to be able to drop this item in Gale Valley from a random Ore Vein. Ending the campaign will unlock this quest and the Righteous Idol one it seems and the last aspect dungeon of the Peaks.

This doesn’t seem to drop randomly anywhere. I have dozens of hours in fractured peaks. But it dropped on the first bush i harvested in the “Pallid Glade” subzone just west of Nostrava WP.

The northern part of Pallid Glade is frozen and has almost zero plants. The south is warmer with less snow. I got the Frostbloom at the lowest area on a road just at the border of the Dry Steppes.

I’ve done Icebound Geode and Righteous Idol, and am stuck at 33/35. There are no indicators what I haven’t done in-game. I did all of the Zalan Coste random drop quests. What am I missing? There are 0 side quest indicators for me in that zone. I spent over an hour in Pallid Glade last night in case it was Frostbloom or Hungering Bone Cache, but nothing dropped, so I assume I did those a while ago. I wish this game had a better tracking system of what is incomplete. The menu in the game doesn’t track everything. Ughh.

I was missing Frostbloom and Bloodied Note.
Frostbloom took days to farm and finally drop.
Bloodied Note must have dropped out of my inventory when I went to town after originally getting it. That is a qol change I think the game needs to make, to stop dropping zone specific items, as this whole time I thought that quest was done when it really wasn’t.

Thank You, Icebound Geode was missing for me. The problem with quest items is that the mechanic of dropping those items from dungeons , to prevent clutter , has extended to open world and drops quest items. People are unaware of this mechanic as was i until i noticed quest item drop when I have used teleport

About “Malady of the Soul”: The text says it’s available after progressing act 1 (which I assumes means the act quests, the ones in yellow), but that’s not what unlocked it for me;
I was fully done with act 1 and it still wasn’t there.
It unlocked when I hit level 20, while doing the Sanguine chapel for the quest.
So I’m guessing one of these two things (clearing the sanguine chapel or reaching level 20) is what unlocks this quest!

Season 2 added 3 new side quests in Fractured Peaks. They are standard side quests, available in Eternal realm too, they are not seasonal.
