For trophies & achievements you need around 85% game completion. The only collectible types not required are: Resistance Field Labs, Memory Paintings, Ikran Gear Baskets. Of everything else you need either all or some. For 100% Air Quality you must destroy all RDA buildings, indicated by the brown color on the map. Everything else are classic collectibles you pick up in the open world. Most of these get marked when unfogging the map by purple/blue dots. What you found will have green dots on the map, this makes it easy to keep track.
There are a few missable Audio Logs & Notes from story-related areas you can’t return to after the story. However, for trophies/achievements you only need 30 of these and there are more than that in the open world. Hence none of the trophies/achievements are missable. Even if you miss everything possible, there are still enough Audio Logs & Notes after the story. All other collectible types remain fully available after the story in free-roam.
The Resistance HQ becomes unavailable after “Main Quest 20: Victory Celebration”. If you missed the collectibles from Resistance HQ they respawn at new locations, so these aren’t missable. Some Side Quests from the Resistance HQ become permanently unavailable but these don’t matter for trophies/achievements. If you strive for 100% completion you should do everything from the Resistance HQ at the first opportunity when it spawns. For trophies it doesn’t matter. All trophy-related side quests and collectibles are still doable after the story.
Collectibles can be tracked via the world map, completed ones have green dots. Some can also be tracked from the Journal under Lost Items / Audio Logs / Notes. On PS5 you can track progress via the trophy list too.
The best approach is to play through the story until you can use the flying mount in all regions. This makes exploration easier. Collect any Audio Logs & Notes you find duringthe story. Then after the story clean up all remaining collectibles.

Thanks for putting them all together. Hopefully someone will find the dolls soon, too. 🙂
