Blood Ties is Chapter 14 in A Plague Tale: Innocence. This walkthrough will guide you through all objectives of Chapter 14 “Blood Ties”.

Part of Amicia’s dream was real. Hugo has in fact been actually captured by the Inquisition, but lured by a rat Hugo finds a way to escape. In fact, in the chapter you’ll play as Hugo in a what it is a huge stealth portion of the game happening across the headquarters of the Inquisition. Hugo cannot rely on an inventory of ammunitions. If someone finds him, the alarm will be sounded immediately. Additionally, for 90% of the chapter you need to constantly be in crouch mode, otherwise you’ll get spotted. Fortunately, this chapter has many checkpoints.
Find Beatrice
Start by interacting with the cabinet using to crouch underneath it.
Exit through the door and crawl through the crack in the wall.
Quickly hide behind the fence indicated by the arrow below and when the monk operates and lifts the machinery, go through the created opening.
Next, hide behind the crate by the wheel and wait until the monk operating the quern doesn’t see you before continuing behind a column.
Then, hide under the table and wait until the two monks have finished talking before moving.
Once you’ve reached the other side of the room, hide under the book cart for just a split second to make sure no monks are patrolling and then turn back around. Here, try to avoid hitting the glass bottles on the floor and then keep going through a new hidden corridor.
Again on the other side, this time wait for the monk to walk by and then go up the stairs.
Upstairs, wait for the monk to grab the distillate barrels before moving and the continue on through the bottles. Then, turn right to find another monk working with some crates and quietly access the bridge bringing to the distillate mixing area.
After the bridge, turn left to avoid the standing guards and go back around the first big pool.
Then, wait for the guards to finish talking and then continue on to reach the third pool of distillate. Here, right after the small set of stairs, head left to avoid coming across the guard and the monk.
Be careful not to trip on the bottles, make your way around the pool and the stairs and then go upstairs one more time.
Here, you need to take advantage of the white smoke to move from one furnace to the next.
When you’re in the room with the long table, veer left in order to be perpendicular to the door found in the next room. This will prevent you from being caught by the walking guard coming out of the door.
Run across the other side of the cathedral and enter the only available doors.
Hide under the book cart when the monk doesn’t look, then wait for him to walk to the other side of the room before crouching on under the cabinet.
In the next room two monks are talking about Hugo. Wait before the standing monk turns his back on you, then go hide under the book cart and enter the door to continue undetected.
In the next room, hide behind the ledger and then one of the book carts. Wait until the walking monk go in the opposite direction and then go hide under yet another book cart on the left of the room.
Continue on behind the tables with the red tablecloths and then again hide under a book cart adjacent to one of the two tables. Wait a split second and then enter the new room in front of you.
In the next area, immediately hide under the book cart and quickly inside the cabinet on your left.
From the cabinet, proceed under the table and then immediately to the adjacent cabinet on the other side of the room. We’re safe for now.
In the new area, walk towards the natural light shining through the windows.
A new cutscene will play out during which Vitalis, while meeting with Archbishop Gauthier, injects himself with Hugo’s first threshold blood. Lord Nicholas joins the meeting and warns Vitalis that Hugo somewhere around the cathedral.
Find Nicholas to Beatrice’s Cell
Stay close to Lord Nicholas, but not too close. In the first area with all the other guards, immediately go right to hide behind some benches, making sure that none of the guards spot you.
Wait until Lord Nicholas is done talking with the guard, then hug the wall to remain unspotted and proceed at the same time. Keep following your target until you get to a garden.
Now, here we’re first going to go for the only collectible (Herbarium 11 – Black Nightshade) available in this chapter and then make use of the reload checkpoint feature to take care of the chapter-specific trophy Captain Sidekick. For both the collectible and the trophy, you can use their respective pictures to make this easier. Now, to get to the collectible, go the opposite way as Nicholas, making sure you always crouch.
Then, make your way across the garden using the straight path shown below. At the end of it, don’t turn right into the north path of the garden, instead use the opening in the hedges to avoid being spotted by the guard.
Crouch to the next opening in front of you and head left, then right around the hedge.
Then, turn right and around the same hedge to find a new opening. Sitting at the end of this small hedge area is the collectible.
Restart your checkpoint. Now, we’re going to unlock the trophy Captain Sidekick. The trophy description is misleading because what you have to do is exactly the opposite. Instead of staying with Lord Nicholas, you have to get ahead of him, then cross the door he would walk through first if you did stay with him, and hide under a table. This is possible because Lord Nicholas stops to talk to a monk for a moment, giving us a quick opportunity window. Now, to get to the door before him, take the same exact path you took for the collectible, but this time you need to turn left at some point. Remember that Lord Nicholas talks to the monk for just a couple of seconds, so you may want to run here a bit (do NOT use or you’ll be spotted). After turning left (use the pictures below if you need help), enter the new area, walk across the room, and hide under the table to make the trophy pop.
Now, if you’re lucky you can even make it under the table right next to the one you’re under and not be spotted by the standing guard. If this is the case, you’ll immediately find yourself in the prison area, just keep going straight on until you reach Beatrice’s cell. Remember that in order to initiate the cutscene during which you talk to Beatrice, you still have to wait for Lord Nicholas to catch up with you and for him to talk to Beatrice first. If you do get spotted, you should spawn back in the garden. This time, let Lord Nicholas go through the door first. For this walkthrough, we’re going to assume that you did get spotted by the guard and continue with Lord Nicholas in the lead. Use the same path we used to unlock the trophy, but this time make sure Lord Nicholas is in the lead. He will shut the door to the room we went into before for the trophy, forcing us to find another way to the prisons.
Walk up the stairs until you reach a corpse dissection room full of monks. Use the pictures below to know where to go and not get spotted.
In the next room, use the stairs on the left to hide behind some rat cages, ignore the prisoner, remain hidden under the cart and go crawl through the hole in the wall when the monk and the guard don’t see you.
In the next room with all those kneeling corpses and the walking monk, head right to rejoin with Lord Nicholas follow him again.
He will shut another door behind you, but fortunately for us there’s a crack on one of the walls for us to crawl through.
In the new room, wait until the two guards leave the area, then continue on until you spot Lord Nicholas again. Wait before Lord Nicholas finished talking to the prisoner before following him again.
At the end of the alleyway, Lord Nicholas will be talking to Beatrice who doesn’t seem to be much collaborative. Remain hidden as Lord Nicholas makes his way back, giving us a chance to talk to mom.
Approaching Beatrice’s cell will start a cutscene during which Beatrice asks Hugo to retrieve the key that unlocks the cell.
Find the Key to Beatrice’s Cell
Crawl through the crack in the wall next to Beatrice’s cell and wait for the guards to spread before moving on.
Approach and climb the ladder on the right side of the area and walk on the overhead planks.
The path here is linear and will lead to a second ladder. Climb down and activate the lever to free the rats and scare the guards away.
After the area’s clear, we can go get the key, but the now there’s rats all over the place. Use the beam of light next to the first ladder and point it at the other beam of light. Then, use the latter to point it at the key hooks to grab what you need.
Go Back to Beatrice’s Cell
Now that you’ve got the key we need to make our way back to the cell. Redirect the beams so that the path ahead is cleared of rats, then crawl back under the crack, and finally interact with the cell.
A new cinematic will start during which Hugo and his mom share a reunion hug.
There’ll be time for more hugging and kissing later, it’s time to escape now, but not without first learning about the Prima Macula’s real power. Hugo hears voices in his head, sign of the fact that Hugo is way beyond the First Threshold. In order to escape, Beatrice urges Hugo to reply to the voices in his head.
Find the Exit to the Bastion
Hugo is now capable to talk to and control rats. Ah, if only Hugo replied earlier…! A new power is unlocked: Imperium. To use it hold to aim and then hold to send the rats to the designated area. With Hugo’s new power, we’ll be able to make it out the prison. Use Imperium on the first group of rats and walk backward to easily maneuver the rats and move around. Every time you reach a source of light, indicated by red paint not the floor, you can let go of Imperium.
When you reach the first light source, turn around and have the group of rats in front of you make the guard leave. Then, let go at the next source of light and keep making your way out. In the next area, you’ll see a guard in the middle of a brightly-lit corridor. Because of this, we can’t direct the rats to it. Go right instead, and use Imperium on the rats one more time to make it across the room alive. Then, do the same thing in the next room.
In the next area with the guard by the table, move the beam of light to your right to liberate the rats and direct them at the guard. The guard will run away in fear.
Exit the area and keep using Imperium on the rats to create a path forward. Keep using Imperium until you reach a long corridor.
When you reach its end a new cinematic will start. Vitalis, despite now having the Macula in his blood, wants to keep Hugo for himself, at least until he has reached the Final Threshold. Vitalis wants to reach it, too, but needs Hugo for it. Lord Nicholas wants to put an end to Hugo and Beatrice, but Vitalis wants them alive for his evil purposes.
Defend Beatrice
To get through this final section alive, Hugo must use his newly discovered power to get rid of the incoming guards and defend Beatrice. For this whole section, simply use Imperium on all guards, taking extra care for the archers on top of the stairs who can easily kill you with a well-aimed arrow. It’s also important to keep turning your camera around as enemies come from all sides.
After a while, Lord Nicholas orders the monks to call the rats back to their cages, with Hugo exhausted on the floor. All efforts to defend Beatrice have been in vain though. Vitalis keeps Hugo and Beatrice as his prisoners.

The game is great! and the walkhrough is very helpfull
The only thing that bothers me is this chapter, they want to make it seem like Vitalis is some kind of bad seed within the Church and they are trying to do something to stop it, and thereby clean up the image of the Catholic Church
It is widely known that the inquisition was the policy of the Catholic Church to combat “heresy” and that is why it was carried out systematically in many countries.
They were Nazis killing people who did not believe the same as them or obey their laws.

It’s a video game that makes the rats a hive-mind zombie army and the sci-fi element of Hugo being able to control them. Expecting an accurate historical representation of the church and inquisition is a bit silly at this point in the game.

Found out the hard way: If you wish to get the Sidekick achievement, you MUST make it across the hallway to the room where you hide under the table as seen in the guide on your FIRST TRY because there is an auto save as soon as you cross the threshold after exiting the garden. If you fail, it will load you right behind Lord Nicholas and you will have to restart the entire level.

You can still actually get the trophy then stuck directly behind him by running straight past him and into the door. It took me a lot of tries but I just did it because I didn’t want to start the level again

hugo controlling rats to kill soldiers and save mom is impossible, its so unresponsive, the rats take too long or many times dont respond at all and it has become extremely frustrating, I’ve attempted this 20 times and I simply cannot get through this part, This is a major reason scripted portions of games without halfway save points are usually deleted or thrown in the trash.

Not sure what platform you’re on but did this level this morning, first try, simply by holding both L2 and R2 all the time and just moving that yellow halo with the right stick towards the red warning ‘droplets’. The rat hive was just constantly following my right stick movements engulfing anything in their path. Took about a minute, honestly. I’m on PS5
