All That Remains is Chapter 12 in A Plague Tale: Innocence. This walkthrough will guide you through all objectives of Chapter 12 “All That Remains”.

Look for Medicine in Hugo’s Wing
Hugo’s getting sicker and sicker, but to complete the remedy Lucas needs a bigger laboratory with better equipment. Amicia suggests they try with her mom’s laboratory at the estate, so that’s where they venture. When Amicia and Lucas get there, the main access bridge is no longer practicable. Fortunately, though, there is a side path to the right. Take this path and cross the river using the fallen trunk.
Lucas then notices how the rats are weirdly behaving and not attacking them. Why? We’ll find out soon enough.
Keep walking uphill and when you reach a torch, turn right. The main path cannot be crossed because of a horde of rats. Keep going until you see a cart with a brazier blocked by a chain. Equip a rock to let the cart roll down and eliminate the rats. Now, head back to the main path which is now safe to use.
Before moving on, we need to grab a collectible (Gift 11 – Calamus) found within the house in the middle of the upper area. Cast Odoris as shown in the picture below to temporarily magnetise the rats to it and then use Ignifer on the nearby embers to avoid them from getting in your path.
Then, crawl under the wooden door to find the collectible sitting on the table.
Back out of the house and go left. Here, you can optionally grab some rare materials from the chests inside the small prison area if you use Luminosa to make quick work of the rats. Using Odoris here won’t cut it. Next, given that the door to the right of the area is locked, we need to get to the other side of it to unlock it. Lucas will handle the beam of light for us as we make it across. This section is simple enough: move when Lucas moves beam of light.
Then, quickly run to the next beam of light in front of you, making sure to stay within the light.
Now, equip a rock and aim at the door lock to unchain it. Then, use the new beam of light to create a safe path of light for Lucas to join you.
Next, use the beam of light once again and point it at the dying guard on your left and then quickly run to the next source of light. To make it to the next source of light by the cart, cast Odoris as shown in the picture below and quickly make it across.
This is where we’ll unlock the chapter-specific trophy Merciful. While the first dying man cannot be killed by the rats, the second one by the cart can and it’s on this one that we have to show mercy. To unlock the trophy, you just simply have to avoid directing the rats to him. Do NOT use Exstinguis on the torch above him or else the rats will scurry over to and make quick work of him. Instead, use Exstinguis on the brazier north-west of you and then Odoris in the same area to then quickly cross the area and unlock the big wooden door. The trophy should unlock as you enter the new area.
In the new area, Amicia will spot her dad’s frozen corpse still lying on the ground after having been killed by Lord Nicholas. The rats will, however, devour it shortly after. As soon as the cutscene ends, quickly craft lots of Ignifer ammunitions. We’re going to need a lot starting this area of the chapter.
To be able to survive this section, constantly move your camera around you to see if there are any incoming eddies of rats. If an eddy of rats gets closer to you, they will douse off whatever source of light is left and instantly kill you. To get rid of incoming eddies, equip Ignifer and cast it at them.
When the situation turns back to normal, grab the torch from Lucas and enter the hole in the wall found in the north-east side of the area.
Walk with Lucas through the dining room and when you reach a second room with a servant’s corpse lying on the floor, approach the chest on the right of the room to grab a collectible (Curiosity 21 – Harnois Helmet).
Now make your way until you reach a torch mounting spot. Place your torch here and boost Lucas up. Lucas will help in return.
Go up the stairs to see rats converging into a specific area of the estate. The rats are behaving more and more weirdly and are not attacking our heroes.
Follow the rats and enter Beatrice’s study to the right. Have Lucas interact with the infusions atop the pile of books. Unfortunately for us, they won’t do.
Continue exploring this area by opening the door in the room next to the study. As soon as you enter, a new cutscene plays out. Hugo’s room has been miraculously left untouched by the plague. While Lucas checks other possible blends, Amicia wonders why Hugo’s room is untarnished by the infestation. Lucas doesn’t want to give up on his search for the right elixir and asks Amicia if she knows about a secondary secret laboratory that her mom didn’t want her to see when she was little. As it turns out, somewhere in the estate are Roman ruins that Lucas regards as the perfect place to do alchemy. First, though, we need to get across the estate’s gardens.
Leave the House
As the path you came from is now a rats’ nest, unlock and use the other door to get out of Hugo’s room. Now immediately craft some Ignifer as more eddies of rats will come twirling towards you.
Follow the path Lucas has opened for you and be ready to take down more eddies. Keep following the lights and crouch to get underground. We need to make it across the gardens and reach the barns now.
Find the Medicine in Beatrice’s Laboratory
Before heading down, take a left as shown in the picture below and at the end of this small trail you’ll find a collectible (Herbarium 9 – Cinquefoil) for you to pick up.
Now, you can go down the stairs and stop at the first torch. Craft Ignifer and cast it at the straw’s embers on the cart and the torch in the distance. Then, quickly run to the torch you’ve just lit.
Now, for the next section, your job is to light two sets of straw and a torch in quick succession, so make sure that you have 3-4 Ignifer ammunitions at least. Cast an Ignifer at the first straw, then quickly cast a second Ignifer and the next one and finally a third Ignifer at the torch. Fire on straw dies out quickly, so you need to be fast here. We’re safe for now.
Now, before operating the cart, we need to make a brief detour for an Alchemist Cart (4/5). When you reach the torch, look to the left and you’ll spot the cart. This time getting to it requires a slightly different approach: near the cart is an unlit emitter of beams. Cast Ignifer at it and then cast Luminosa at the rats right in front of you. Immediately run towards the Alchemist Cart. If you do, you should have sufficient room and light to enter the cart and not be devoured by rats. Your progress will be immediately saved after you’ve entered the Alchemist Cart.
To get back to the cart with the brazier on, use the beam of light and point at the location of the cart.
Now, before the next section, I strongly advise you craft at least 10-12 Ignifer projectiles. As Lucas pushes the cart, you’ll need to defend yourselves from multiple eddies of rats, so it’s better to be prepared. Ignite the brazier on the cart and Lucas will automatically start pushing it.
For the next 3 minutes, you’ll need to use all your Ignifer ammunitions to get rid of incoming eddies of rats.
At the end of the trail, Lucas is exhausted for all that pushing but Amicia is grateful for his help.
Find Beatrice’s Laboratory
Now proceed down the stairs and enter the new area.
To get to the Roman ruins, we need to push a crate but if we do so immediately, we’ll be eaten by the nearby rats. Instead, start by dousing the torch you see on the wall using Exstinguis. Then, craft and use some Odoris to concentrate the rats into one spot and then quickly craft and use Luminosa to blow them up. Now, it’s safe to move the crate and continue.
In the next room, approach the light in one of the basins, then turn your camera around and point it down to see a hole in the floor.
Enter it to access and underground area that will allow you to turn the other two lights on and reveal the symbols and, in turn, the solution to enter the laboratory. When underground, follow Lucas’s directions to know where to go. Get a stick and light it using the brazier, then start moving around to see if you’re on the right track, if not, Lucas will tell you. If so happens, go the opposite direction. The goal here is to light two more braziers to reveal all three symbols.
Once all basins are lit, exit the underground tunnels and approach the lion’s marble mask and press to unlock the door and enter Beatrice’s laboratory.
Find the Medicine in Beatrice’s Laboratory
The room is pitch black, but there’s no need to light it up, Lucas will do that for you. Fortunately, this laboratory has vastly superior equipment to make Hugo’s cure. At some point, the whole room will have light. Before doing anything else, let’s grab the final collectible (Curiosity 22 – Family Tree) for this chapter. You’ll find to the right of the room next to a still and a vase.
It appears that Hugo is not the first Carrier of the Prima Macula as depicted by one of the room’s frescoes. Next to Lucas is a box. Approach it and then hit to open it.
Inside is the nearly finished cure for Hugo Beatrice had been working on before disappearing. Thanks to the Sanguinis Itinera and his alchemical expertise, Lucas thinks he’ll be able to complete the cure.
However, as Lucas gets ready to finish the cure, new groups of rats invade the place. There’s going to be many eddies of rats starting now, so seize the opportunity to craft lots and lots of Ignifer ammunitions and while we’re at it cast one at the brazier nearby. We’re going to need it.
Find the Macerate in the Pharmacopeia
Lucas needs a specific ingredient to continue his alchemical experiment. The ingredient is by a white curtain, but is inaccessible because of the rats. Use the mechanism at the centre of the room to move it and halt it by the curtain and then proceed to collect the ingredient.
Amicia doesn’t know which pot to pick. Pick and smell each pot until you find the correct one. For us it was the one on the farthest left.
Bring Lucas the Macerate
Now go back to Lucas to hand him the macerate. As you run back to him, new eddies of rats will surface from the ground, so equip your Ignifer to make quick work of them.
After getting rid of the first eddy, give the Macerate to Lucas.
Defend Lucas While He Prepares the Remedy
Now, keep dismantling the eddies in order to protect Lucas as he completes the remedy.
Create a Path to the Still for Lucas
Now, use the mechanism in the middle of the room to bring the brazier back closer to Lucas, but be careful because new eddies of rats will come after you, so you’ll have to intermittently use the contraption and kill rats to make it alive here.
Lucas will try to help when the eddies are near, but the rest is up to you. If Lucas doesn’t get far from the brazier, it means you’ve haven’t reached the station he’s supposed to work at yet. When Lucas reaches the stills, leave the mechanism, so the brazier protects him from the rats.
Defend Lucas While He Prepares the Remedy
Once again, as Lucas finishes the remedy, use Ignifer on the incoming eddies to protect yourselves from them.
Bring Lucas Back to the Central Alcove
Next, bring Lucas back to the central alcove station, but as he approaches new swarms of rats will come out of the ground. Luckily, Lucas manages to move to a different station to complete the elixir.
Defend Lucas While He Prepares the Remedy
For this final objective, simply take down all incoming eddies to get you and Lucas to safety.
After a short while, Lucas will finish the elixir and the rats will stop attacking you and Lucas. Again, weird! The elixir seems to give Amicia and Lucas invincibility powers as they can now easily walk through the rats and not get attacked.
Back at the Chateau, Amicia and Lucas immediately administer the Elixir to Hugo which will slow down the harmful effects of the Macula and its Thresholds. Hugo, now knowing that Beatrice is alive, wants to see her, but Amicia, in order to protect him, continues lying to him. Oh, Amicia!

I’m extremely stuck in Ch. 12 whilst going for Merciful. The technique just doesn’t work for me, even if I throw then run INSTANTLY I still die as I’m opening the door for Lucas.

I’ve played through Chapter 12 multiple times and the technique explained above is the one that works the best to get across the area alive and still unlock the trophy. What exactly are you trying to do?

You can also choose to skip the Extinguis cast and Odoris right in front of the door then immediately Luminosa the pack of rats and that way you get get all the mats by the brazier too!

The first dying soldier absolutely can be killed by the rats. Move the light beam away from him, and then throw a stone at the lamp above him. Amicia will remark on what just happened when you get to the following guard.

if u don’t have luminosa throw extinguis onto the torch near the rats, then use odoris to attract the rats to the door way, after they’ve all gathered there light the torch again to keep them trapped. you’ll be able to move the box without getting eaten, then there will be leather on the other side so you can craft luminosa to kill the rats

When you have gotten ready to move on from the area by the second soldier at the workstation use an ordoris near the house door and move to the second soldier in the light. Then use an exstinguis to knock out the flame in the brazier by the red tent you just came from, followed immediately by another odoris into the red tent drawing all the rats inside and quickly relite the brazier trapping the rats inside the tent. From there you have all the time in the world to go through the door of the house.
