Crash Bandicoot 4 It’s About Time has 21 Flashback Tape Locations. Each Flashback Tape unlocks a Flashback Level, which are small 2D levels. Finding all of these tapes doesn’t unlock a trophy or achievement directly, but they are needed to get the Platinum Flashback Relics and 106% completion ending.
Not every level contains a Flashback Tape. You can see which levels have them by opening the Level Stats Menu. At the bottom it will show a grayed-out Tape Silhouette. When you found the tape it will appear in this menu. This way you can keep track which tapes you’ve already collected. None of them are missable, all levels can be replayed after the story.
In order to get the Flashback Tapes you have to reach them without dying. Make sure you finish the level afterward to make the Tape count (don’t quit the level). It’s okay to die AFTER picking up the Tape. Just in the parts leading up to the tape you can’t die. If you die, the tape will become grayed-out and cannot be picked up. That’s what makes this a little more tricky (especially in the later levels).

Flashback Tape #1 – Crash Compactor
At the very start of the level.
Flashback Tape #2 – Hit The Road
At the entrance of the bonus path (“?” platform).
Flashback Tape #3 – Booty Calls
Close to the start of the level, just after the first Checkpoint Crate on the left.
Flashback Tape #4 – Jetboard Jetty
At the entrance of the bonus path (“?” platform).
Flashback Tape #5 – Give It a Spin
Just after the first Checkpoint, before the first water lilies you must jump on.
Flashback Tape #6 – Draggin’ On
At a section with 3 “stone cats” enemies, under a golden gong (about halfway through the level).
Flashback Tape #7 – Off-Balance
In the first section with dragon statues that spew fire.
Flashback Tape #8 – Off Beat
Where the 2nd Checkpoint Crate is, on the right side of it.
Flashback Tape #9 – Run It Bayou
While riding on the rooftop of the boat it’s in mid-air.
Flashback Tape #10 – Snow Way Out
When you first reach some ice quadrants in water that you must jump over, the flashback tape will be in plain sight on the main path. You’ll walk right into it, practically impossible to miss it.
Flashback Tape #11 – Stay Frosty
Near the end of the level, 2 checkpoints AFTER the rail grind of this level. You’ll have to pass a 2D section by jumping on ice in the water. After crossing this ice the Tape is in plain sight on the main path.
Flashback Tape #12 – Bears Repeating
During the polar bear riding section, on the right side during a jump.
Flashback Tape #13 – Blast To The Past
After the long rail grind, in the section where lava is shooting from the ground.
Flashback Tape #14 – Dino Dash
Quite late in the level, after being chase by the T-Rex for the 2nd time, it’s at the beginning of a lava section.
Flashback Tape #15 – Out For Launch
Quite late in the level, after a rail grind that has lots of Nitro Crates, in front of an enemy with a laser firearm.
Flashback Tape #16 – Stowing Away
When you find the purple mask ability in this level for the first time, the flashback tape will be at the ceiling right in front of you (spin into it with the purple mask ability).
Flashback Tape #17 – Crash Landed
After getting the 2nd green mask pickup in this level (not counting the one inside bonus path), the flashback tape will be in plain sight on the main path where you have to walk upside down. Basically unmissable.
Flashback Tape #18 – Food Run
In one of the sections with moving cars that have arrow-walls on their sides, the flashback tape is floating over some boxes. Also in plain sight on the main path. You’ll be using anti-gravity in this section. You must disable the anti-gravity for a moment to drop down onto the boxes with the tape.
Flashback Tape #19 – Nitro Processing
Before the last rotating water wheel in the level. Close to the very end of the level.
Flashback Tape #20 – Toxic Tunnels
During the last checkpoint of the level (very close to the very end of the level). Before a rotating platform with a shielded rat enemy on it.
Flashback Tape #21 – Cortex Castle
During the difficult laser grid section at the end of the level. While using the Inverted/Anti-Gravity ability.

Is anyone have problems with transparent tapes and not being able to collect them. The tape in “off-balance” is transparent and it won’t let me get it. Am I missing something that I need to do first?

You have to reach the tape without dying (this is pointed out in the intro section of this guide). If you died, it becomes transparent and cannot be collected.
