MediEvil (PS4 Remake / Remaster) has 9 Life Bottle Locations. They are collectibles that increase your health. Finding all of the Life Bottles unlocks the Healthiest Man Alive – Er Dead trophy.
Some of the Life Bottles are located in specific levels. Others are rewards for redeeming Chalices in the Hall of Heroes.

After finding a Life Bottle it’s important you finish the level to save the progress. If you get a Game Over or quit to world map it won’t count.
None of the Life Bottles are missable. You can still get all of them after the story via level select.
Life Bottle #1 – Dan’s Tomb
Inside the tomb where the game starts, to the left of the gate you must open the leave the tomb. A mini-cutscene will point it out to you.
Life Bottle #2 – The Graveyard
When you reach the angel statue, you must hit the statue to make it face left. Then the left gate opens, behind it you find the Life Bottle on a hilltop.
Life Bottle #3 – Hall of Heroes
Reward for redeeming the 2nd Chalice (give it to the first statue on the left in Hall of Heroes)
Life Bottle #4 – Dan’s Crypt
After beating Level 5 “Return to Graveyard”, return to Dan’s Crypt. On the right side of the crypt, there’s a part of the wall that looks brighter than the rest. You can use the dash ability that you unlocked at the end of “Return to Graveyard” to break down the wall. You can also use the Hammer or Club to smash the wall.
Life Bottle #5 – Scarecrow Fields
In the last spinning trap before the end of the level, there’s a secret side path leading to a Harvest Part. Pick it up, then backtrack all the way to the locked barn in the fields, just before the trap section (but after the 2 Mecha Imps). Destroy the chest behind the barn to release a serpent that kills the enemies for you, and then kick the cart behind the barn. This lets you enter the barn through a crack in the wall where your serpent friend killed 2 scarecrows for you. Jump up the haystacks to reach the lever on top, release the lever to drop the harvest machine, stand near it and insert the Harvest Part. Now the machine will mow down a path outside, at the end of that path you can find the Life Bottle along with this level’s Chalice.
Life Bottle #6 – Pools of the Ancient Dead
After opening the red rune door, on a small island on the right side. You can jump over to it. The green life bottle is very visible from far away.
Life Bottle #7 – Hall of Heroes
Reward for redeeming the 16th Chalice (centaurus statue upstairs).
Life Bottle #8 – The Time Device
After turning off the laser grid to grab the green rune, backtrack to the start of the level where the clock puzzle is. On the right side of the starting area is a locked path (locked by a butterfly “door”). When you walk to the butterfly door, to the left of it a clock will appear that shows where to put the golden and the silver clock handle. You must do this on the clock puzzle platform to open the butterfly door. Then you can access the green life bottle at the end of that path. At the start of the level it would be shielded under a red laser field, but because you turned off the laser grid the bottle is now accessible to you.
Life Bottle #9 – Hall of Heroes
Reward for redeeming the 20th Chalice (statue behind door halfway up the stairs).
That’s all 9 Life Bottles / Health Upgrades in MediEvil (PS4 Remake / Remaster).

Thats pretty messed up. I collected all chalices but after the Ship level, my game crashed inside the hall of heroes. I carried on playing and I am now in the hall after I got the chalice in the Chronograph level. I got 20 chalices and both trophies (Life bottles and all chalice rewards).
So, the game crashed after the ship (no reward from the shield guy. I think it was him glowing), after the Entrance halllevel I got some coins as reward. Now, after the Chronograph I got that sword guy glowing and he gave me three energy bottles, then the female statue behind the door started glowing and got me the 9th life bottle. I think my game was messed up now because you should not quit inside the hall of heroes. Glad that it still worked and got both trophies!

There is a second life bottle in Dans Crypt by breaking down the wall to the right facing the exit. It also has two treasure chests.

Hi , I finally collect all the 20 chalices but I didn’t collect the last reward in hall of the heroes and continued to last level ( Zarok’s Lair ) and finished the game. So, the problem is I can’t go back to Hall of The Heroes to collect my rewards .. Do you have any idea or answer for my problem..?
